At BCBSAZ Health Choice Pathway (HMO D-SNP), we value member voice and choice. What does that mean? It means all members have the right to voice their opinions and choose how they receive healthcare services to meet their needs.
The committee’s purpose is to get feedback from our members – like yourself! You can make a difference by sharing your experiences as a member of our health plan and helping us make improvements.
We’ll cover topics like:
- Health plan benefits and programs
- Healthy Rewards program
- Community Resources and more!

Join the Cultural and Diversity Council
Culture shapes how we act, think, and communicate. It also has an impact on how health needs are being met. Help us learn more about you and your community. This way we can improve how healthcare services are delivered to you and others.
By joining this council, you will help us find ways to improve:
- Our projects and communications to help promote care that supports your culture
- Member-and family-centered care
- How we work with our providers, doctors, agencies such as community centers and resource agencies, and communities to meet member needs
Meetings last one and a half hours, every month. During these meetings, you will:
- Share experiences
- Suggest improvements
- Gain understanding of cultural competency
- Receive training two times each year.
Meetings and trainings are remote at this time.

Rental Assistance Is Available To Help Renters and Landlords
Renters and landlords in need of financial assistance can use a new website, consumerfinance.gov/renthelp, from the Consumer Financial Protection Bureau to find rental assistance providers in your area.
If you know of a renter who’s having trouble paying rent, utilities, or other housing costs – or a landlord trying to stay afloat with tenants in this situation – help may be available. State and local programs are distributing billions of dollars in rental assistance to help renters stay housed during the pandemic.
Visit the CFPB’s Rental Assistance Finder to find resources for renters and landlords to help navigate various financial hardships related to the pandemic.
