Did you know that Arizona is ranked thirteenth in the nation for people with diabetes? One in ten Arizonans are living with diabetes – and an estimated one in three have prediabetes, a condition that increases the risk of developing type 2 diabetes and other serious health problems.1
Diabetes can also affect financial health. The average annual medical cost for a person with diabetes is $16,752. That's more than twice the cost for a person without diabetes.
The good news is that type 2 diabetes and prediabetes are very manageable. With AZ Blue, your district employees can get the care, education, and support they need to manage or reverse the progression of diabetes.
Your employees deserve personalized support. AZ Blue has created a Diabetes Action Plan to reduce diabetes progression by 25% by 2025. How do we plan to do that? Through a comprehensive approach that includes:
- Health management programs: Our registered nurses, registered dietitians, and certified diabetes education and care specialists work with your employees to create and help them follow a personalized care plan. They can also coordinate care through medication management, behavioral health, and social services.
- Dietitian visits: Proper nutrition is key for diabetes prevention and management. In-network dietitians can help your employees learn about their nutritional needs, plan meals, and how to eat healthy. Most of our plans cover dietitian visits with no copay or deductible.
- Prediabetes education and resources: With Sharecare®, AZ Blue’s prevention and wellness partner, your employees have access to a digital health platform with tools, tips, and trackers to improve their well-being and take charge of their health. They can take a prediabetes risk test, sign up for a prediabetes prevention program, and get information and advice on diabetes and related topics.
- Diabetes education: Your employees can visit azblue.com/diabetes to learn more about diabetes and its signs, causes, and treatments. They’ll also find articles, videos, podcasts, and webinars about diabetes care, such as blood sugar testing, insulin therapy, foot care, and eye care.
- Supplemental programs and services: Depending on plan type and eligibility, your district employees can access additional programs and services that can help them avoid or manage diabetes, such as wellness incentives, fitness discounts, telehealth, and pharmacy benefits.
