Did you know Health Choice covers well-woman preventive services?
Preventive services are meant to identify certain risk factors related to disease, behavioral health and the promotion of a healthy life.
Your provider will be screening and counseling you on ways to improve your health and reduce your risk.
It is particularly important that you get an annual well-woman preventative care exam to help identify any issues early and receive the care you need.
There is no copayment or other charges for a well-woman exam. Health Choice Arizona provides transportation to well-woman appointments and can assist you in scheduling an appointment if needed.

A well-woman preventative care exam includes at a minimum the following:
- Physical Exam
- Breast Exam
- Pelvic Exam
- Review of Immunizations, Screenings and Tests.
Some examples of screenings and counseling for a healthy lifestyle include:
- Heathy eating and nutrition
- Physical activity
- Obesity screening
- Oral health care
- Tobacco/substance use, abuse and/or dependency
- Depression screening
- Emotional wellness
- Sexual practices
- Sexually transmitted infections
- HIV screening and counseling
- Disease screening
- Chronic disease management
- Referral to other providers when needed
- Pregnancy spacing
- Family planning services, and supplies
