    Sign up for your online member account

    Access your personalized online member account where you can review your plan coverage, check on the status of your claims, and much more. You can also:

    • Review your plan documents
    • Find and compare doctors or facilities
    • Check the status of a claim

    Your AZ Blue member ID card

    After you enroll in a plan, you will receive your ID card in the mail. It contains important plan details and shows who to call if you have questions. Didn’t get your ID card or need another one? You can request a printed version by calling member services.

    You will need your AZ Blue ID card when you:

    • Visit a doctor or other healthcare professional
    • Go to a pharmacy for the first time to get the medication your doctor prescribes for you
    • Visit an urgent care center
    • Go to the hospital or ER
    • Call AZ Blue customer service

    Find healthcare providers in your plan's network

    The “Find a Doctor” tool lists doctors, other healthcare professionals, hospitals, and facilities that are part of our network. To access it, log in to your online account and click “Find a Doctor.” You will be able to search for a provider in your plan’s network by name, type (area of specialization), or distance from your location.

    It’s always good to find out if providers are in your plan’s network before you see them. Providers who are not in your plan’s network may not be covered by your plan. That means you may be responsible for paying the bill in full.

    You can also call the number on the back of your member ID card to see if a provider you’re planning to see is in your network.

    Complete your annual wellness visit

    Keeping up with wellness visits and checkups is an essential part of staying healthy, and earning rewards for applicable screenings.

    A wellness visit may include:

    • A physical exam
    • Measuring of your vitals (such as body temperature, blood pressure, and pulse)
    • Reviewing your personal and family medical histories
    • Recommended screenings
    • Administering any necessary shots or immunizations
    • Creating a plan to help you improve and maintain your health
    Blue Cross® Blue Shield® of Arizona (AZ Blue) is contracted with Medicare to offer HMO Medicare Advantage plans. Enrollment in AZ Blue plans depends on contract renewal.

    You are eligible to enroll in a AZ Blue Medicare Supplement plan if you are age 65 or older, entitled to Medicare Part A, and enrolled in Medicare Part B, and you live in the plan service area. You must continue to pay your Medicare Part B premiums (and Part A, if applicable), if not otherwise paid for by Medicaid or another third party. During the first six months when you are age 65 and also enrolled in Medicare Parts A & B you cannot be denied a Medicare Supplement plan when you apply for one, regardless of health status.

    Health Choice Pathway HMO D-SNP is a Health Plan with a Medicare contract and a contract with the state Medicaid program. Enrollment in Health Choice Pathway HMO D-SNP depends on contract renewal. Health Choice Pathway Member Services can be reached at 1-800-656-8991, TTY: 711, 8 a.m. to 8 p.m., 7 days a week.

Hours are 8 a.m. to 8 p.m., Monday through Friday from April 1 to September 30; and 7 days a week from October 1 to March 31. Member Services also has free language interpreter services available for non-English speakers.
