Integrated Care Management

The Blue Cross Blue Shield of Arizona (AZ Blue) Integrated Care Management program is here to help when you need it most. Clinical Care Managers work with you to make sure you have the care and services you need if you’re returning home after a hospital stay, or have been diagnosed with a serious health issue, need assistance navigating the healthcare system, or need help managing chronic conditions such as asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), depression, diabetes, and heart failure.

The program offers a range of care management services at no additional cost to you.

Our Care Managers take a personalized and holistic approach to helping members, families, and caregivers arrange healthcare services, and support members to become independent in managing their health. They can review your health history and situation, can assess your health and future needs, get you in touch with local resources, collaborate with providers, and answer any questions you have about state and federal programs.

We help make sure you have the care you need, when and where you need it most. We encourage you or your family/caregiver to call 480-937-0409 (in Arizona) or toll-free at 1-800-446-8331, TTY: 711 8:00 a.m. to 8:00 p.m., Monday – Friday from April 1 to September 30; and 7 days a week from October 1 to March 31 to learn more.
