Steps to review or change your plan

  • 1. Review your information.

    Review and update your household information on your Marketplace application to ensure you get an accurate subsidy for 2025.

    2025 subsidy information will be available November 1.

  • 2. Find out your 2025 subsidy.

    The subsidy information in your renewal letter is based on 2024. Call us at 1-844-369-2583 to confirm your 2025 subsidy that will apply towards your premium.

  • 3. Looking for a different plan to fit your needs?

    Call us at 1-844-369-2583 from 8 a.m. to 5 p.m. Arizona time, Monday through Friday. We can review your options with you before December 15, so you can have a plan that starts January 1, 2025.

  • 4. Pay any outstanding premiums.

    You can view and pay any outstanding premiums on your online account or by calling 602-864-4115.

REMINDER: Don’t forget to claim $100 in rewards before December 31, 2024. Complete your annual wellness visit and other free preventive services during the year and we’ll put up to $100 in rewards in your pocket for each person on your plan!*
