事先授權
在BCBS AZ Health Choice ，我們的目標是與您和您的實踐團隊密切合作，以簡化和加快事先授權。我們的簡化事先授權清單中的許多項目僅要求通知。我們的集中式預授權中心旨在簡化流程，從而及時快速批准預授權請求。
需要事先授權的服務請求可以透過安全提供者入口網站直接在BCBS AZ Health Choice事先授權表格上提交，或透過傳真提交給授權部門。所有請求必須包括完整的診斷和程序代碼以及隨附的醫療文件（根據需要）。
為了加快事先授權並為我們的會員提供優質服務，建議為Health Choice會員的所有醫療保健服務選擇簽約或首選提供者。請注意，所有非合約提供者都必須獲得任何服務的授權。所有州外提供者必須擁有或願意取得有效且最新的AHCCCS ID 號碼。 AHCCCS承保的州外護理必須屬於緊急或突發性質。
