Care Management
Some diagnoses require even greater support. Our Care Management team integrates with Health Management to provide greater access to care for those in need. You’ll get a care manager that will work with interdisciplinary teams to increase your continuity of care and improve your quality of life.

Health Management Frequently Asked Questions
- Health Management is a 30-90 day program for those who need support in managing a newly diagnosed or ongoing chronic condition. It provides support and education on areas regarding, but not limited to, primary and related conditions, diet, medications, assessments of social determinants of health, community resources, treatment side effects, behavioral health assessments and general support.
- You will work with health professionals like nurses and dieticians. Health Management is intended to provide you with tools and strategies to manage your condition. This program does not replace your physician's care. Please continue to consult your primary care doctor and specialists as needed.
- Health Management is offered at no additional cost to members who qualify.
