Our approach to care management

"Recently, I followed up on a care management referral for an AZ Blue member who had multiple specialty service needs and was feeling overwhelmed. After listening to the member’s concerns and talking with her provider’s clinical staff, I worked closely with our Pharmacy team to help her obtain the prescribed enteral nutrition formula for home tube feeding. I also coordinated care for the required in-home PT/OT treatment and ensured that all providers were well informed and ready to meet this member’s specific needs. Our member received all the necessary prescription food and in-home services in a timely fashion. The entire family was very happy and grateful for the assistance so they could focus on supporting their loved one in the day-to-day recovery process."
